Serokonversi Pasien Hemodialisis Reguler Di RSUP. H. Adam Malik Medan pada Agustus 2016 – Juli 2017

Abstract

Abstract.Hemodialysis (HD) patients were potentially susceptible to infections of blood borne disease (HBV, HCV, HIV).. The prevalence of hepatitis C in haemodialysis  patients in Senegal, Moldavia, Kuwait, Bosnia Herzegovina, and Peru reached 80%; 75%; 71%; 59%; and 59.3%.This study aimed to determine the prevalence of hepatitis C seroconversion, hepatitisB and HIV and  determine the prevalence of seroconversion based on the length of patients undergoing regular hemodialysis. Retrospective study. The data were obtained from medical records of hemodialysis patients in Medical Record Installation of RSUP H. Adam Malik Medan from the first of August 2016 to July 31th, 2017. Of 242 patients recorded, 213 patients who fit the inclusion and exclusion criteria were obtained.Of the total sample of 213 regular hemodialysis patients, 126 (59.1%) had positive seroconversion and 87 (40.9%) negative seroconversions. Of the 126 patients with positive seroconversion, most were infected with hepatitis C, followed by hepatitis B, and HIV. The percentages are 93.7%, 4.7% and 1.6% respectively. Of the 120 patients who had a positive anti-HCV most 40% underwent hemodialysis more than 12 months. Of the 6 patients with HBsAg positive most 50% underwent hemodialysis more than 12 months. Two patients with anti-HIV positive all underwent hemodialysis 4-6 months.From the results of this study, patients who experienced positive seroconversion more than those who experienced negative seroconversion. Positive seroconversion occurs most often in patients undergoing hemodialysis with duration of hemodialysis> 12 months. So it is necessary to consider the time of screening can be done per 4 months (3 times a year). Hemodialysis patients are heavily infected with hepatitis C and hepatitis B. HCV and HBV vaccinations are recommended before starting hemodialysis treatment

Abstrak. Pasien hemodialisis berpotensi untuk  mengalami serokonversi  terinfeksi blood borne disease  (HBV, HCV, HIV). Prevalensi hepatitis C pada pasien HD di Senegal, Moldavia, Kuwait, Bosnia Herzegovina, dan Peru berturut-turut mencapai 80%; 75%; 71%; 59%; dan 59,3%. Penelitian ini bertujuan untuk mengetahui distribusi serokonversi hepatitis C, hepatitis B dan HIV  dan mencari prevalensi serokonversi berdasarkan lama pasien menjalani hemodialisis reguler. Menggunakan studi retrospektif. Data diperoleh dari catatan medik pasien hemodialisis di Instalasi Rekam Medik RSUP. H. Adam Malik Medan periode 1 Agustus 2016 – 31 Juli 2017. Dari 242 pasien, didapatkan 213 pasien yang memenuhi kriteria inklusi dan eksklusi. Sebanyak 126 pasien (59,1%) mengalami serokonversi positif dan 87 pasien (40,9%) mengalami serokonversi negatif. Dari 126 pasien dengan serokonversi positif, paling banyak terinfeksi hepatitis C, lalu diikuti hepatitis B, dan HIV. Dengan persentase masing-masing adalah 93,7%, 4,7% dan 1,6%. Dari 120 pasien yang mengalami anti HCV positif yang paling banyak 40% terinfeksi dengan lama hemodialisis >12 bulan. Dari 6 pasien dengan HbsAg positif yang paling banyak 50% terinfeksi dengan lama hemodialisis >12 bulan. Sebanyak 2 pasien dengan anti HIV positif semuanya menjalani hemodialisis 4-6 bulan. Dari hasil penelitian ini  pasien yang megalami serokonversi positif lebih banyak dari pada yang mengalami serokonversi negatif. Serokonversi positif paling banyak terjadi pada pasien yang menjalani hemodialisis dengan lama hemodialisis >12 bulan. Sehingga perlu dipertimbangkan masa skrining dapat dilakukan per 4 bulan (3 kali dalam setahun). Pasien hemodialisis sangat banyak terinfeksi hepatitis C dan hepatitis B. Maka dianjurkan pemberian vaksinasi HCV dan HBV sebelum memulai perawatan hemodialisis.
